Statement of Informed Consent for Usability Test

A group of Masters students in the School of Information Management and Systems at UC Berkeley are conducting a study to assess the usability of the interface of a web-based student discussion forum. If you volunteer to participate in this study, you will be asked to work with a prototype of the forum, navigate through the system, enter some information into the system, evaluate it, and answer some questions. Some questions will be asked in a written questionnaire provided at the end of the session, others will be asked orally before, during and after the session. Your interactions with the system will be recorded through written note-taking.

There are no direct benefits to you for participating other than what may be an educational experience. There are also probably no risks to you; you will simply be using a web-based application, entering some information, and viewing and evaluating the results. We will associate an ID number with your experience and will keep a record of the link between your name and ID number. However, we will not name you if and when we discuss your behavior in research publications.
